- Patients with diabetes seen for the first time must have a primary care referral.
✔ We are a provider for the following insurance companies: Aetna, BCBS, Coventry, Health Net, Humana, Medicare, United Healthcare. If you do not have insurance, you may pay with a major credit card, checks, or cash. Payment is expected at the time of service. Please see our payment policy HERE.
✔ The cost of your visit will depend on the complexity of your case and type of insurance coverage.
✔ Your medical information is available to you through an online patient portal. We encourage you to use the portal to stay up-to-date with your medical services. The patient portal is accessed at the following website: 16181.portal.athenahealth.com
✔ Before your visit with the doctor, please write down questions you might have. Write down the list of all medicines you take including those not prescribed by a doctor.
